2. Required Documents:

   Along with the completed application form, please provide the following documents:

   • 2 passport-sized (2x2) pictures
   • 2 valid IDs (Accepted IDs: SSS ID, Police Clearance, NBI Clearance, Postal ID, Passport, Driver's License)
3. Accepted Proof of Income (if opting for Financing):

   Depending on your employment status, please submit one of the following:

   • For Employed: Certificate of Employment OR 3 months' payslip
   • For Self-Employed: Mayor's Business Permit OR Barangay Business Permit OR Picture of Business with Declaration of Income
   • For Recipient of Remittance: 3 months' latest remittance slip OR Statement of Account/Bank Statement
   • For Pensioner: Statement of Account/Bank Statement
   • For OFW/Seaman: Return Plane Ticket OR Latest Contract
